Event Description
The convening’s theme focuses on the progress made to date, celebrating successes, and being transparent about challenges. We will analyze PAT (baseline and follow-up) and Results data to identify remaining phase two opportunities and identify ways we can operationalize care management activities at the organizational and practice level. During the convening participants will have the opportunity to craft and refine an improvement story as a tool to motivate change. The meeting will also include breakouts on coaching, leadership, and engaging specialists. We expect this meeting to build on the great foundation established during the January, April, and July in person meetings.

Objectives

  • Celebrate achievements and contributions of individual contributors and PTI organizations.
  • Learn about 2018 PTI TA offerings and how to participate as a co-designer.
  • Analyze PTI PAT (baseline and follow up) and Results data and identify remaining phase two opportunities.
  • Discuss strategies for completion of follow-up PATs during Q4 2017.
  • Develop a plan to support the care team in consistently providing care management for patients at the highest risk of hospitalization and/or complications and standardizing documentation of the care management plans.
  • Exchange ideas, best practices, & feedback with peers through facilitated affinity group break-outs & informal connections.
  • Hear from CMS faculty about the Quality Payment Program and its impact on improvement work.
  • Craft an improvement story to motivate change at the organizational and/or practice level and practice giving peer feedback.
